To make it as easy as possible for our league members, we have implemented an
online form to automatically fill out and digitally sign the Medical Release Form. This form needs to be filled out for each participant, each year. After filling out the form and receiving your PDF, please upload the PDF to your child's Participant Info. Please also print a copy of the form and turn it into your coach, even if you have scanned it into your account for this year. All coaches need a hard copy on hand at all activities, in case there is an emergency.
To upload the Medical Release Form to your child's Participant Info, follow the instructions below or in
this PDF.
Log in to your account:
Click on the Edit icon next to the player's name:
Click on the Upload File Document button next to Medical Release Form under the Participant Documents and Forms heading:
Click Choose File and locate form on local drive (note file types allowed):
After selecting the file, click Upload.
If the file uploaded successfully, the file icon and name will display.
Finally, scroll to the bottom of the page and click the Update button.
